Completed applications must include all of the following:

• Curriculum Vitae
• Documentation of obstetrical procedures performed during residency
• 2-3 letters of recommendation from supervisors of obstetrical training (a minimum of one letter from core faculty of applicant’s Family Medicine Residency program)
• ABFM Board Examination scores, USMLE Step 3 scores, Comlex or ABFP In- training Examination scores from the second year of residency if still in training
• Application form
