At the completion of this block, the resident will have developed an understanding of:
- How health systems interface with wrap-around services in the community NOTE: There is a smaller element of this topic covered in the PGY-1 “Practice Management and Public Health” rotation required of all residents. This block is meant to be a deeper dive into those organizations and related topics.
- How community support agencies rise and fall based on grant dollars (usually)
- The grant process from start to finish
- How success is measured and presented in community organizations
- Who are the key community health organizations in the South Bend area?
- How does the physician (and the office support team) act as an intermediary (and advocate) between the patient and community support organizations
- Who oversees the work of community health organizations
- Community Health Needs Assessment
- The interface of primary care, community health and the local Health Department
Residents should ideally:
- Meet with the leadership of as many community support organizations as possible
- Meet with those within Beacon charged with supporting, integrating and working together with community organizations
- Meeting with the Beacon Health Foundation to understand their role in supporting community health programs
- Meet with and perhaps shadow the E. Blair Warner LCSW
- Meet with and perhaps shadow another community health navigator
- Meet with the St. Joseph County Health Officer (+/- Deputy Health Officer), the Chair (or designee) of the St. Joseph County Board of Health, and at least one key staff member of the St. Joseph County Health Department
- As appropriate, meet with those outside of Beacon doing similar things in their organization
- Meet or talk with individuals in organizations that they foresee themselves working with in their early career
- Discuss this topic with regional or national leaders in this field as they may be available through faculty or resident connections
- Do any assigned directed reading
